PHOTOGRAPHIC CONSENT AND RELEASE FORM
I hereby authorize Rhode Island School of Design and those acting on its behalf (collectively, “RISD”) to:
- Record my or my dependent's image, likeness, voice, and/or art works created in connection with this registration on a photographic, video, audio, digital, electronic, or any other medium;
- Use, reproduce, modify, exhibit, and/or distribute any such recording, in whole or in part, in any medium now known or hereafter developed (including without limitation print publications, internet, social media), and for any purpose that RISD may deem appropriate, including without limitation promotional or advertising efforts; and
- Use my or my dependent’s name in connection with any such recordings or uses.
I understand that I shall have no right to inspect or approve any such recordings and uses and that all such recordings and uses, in whatever medium, shall remain the property of RISD. I release the College and those acting pursuant to its authority from liability for any violation of any personal or proprietary right I may have in connection with all such recordings and uses. I have read and fully understand the terms of this release.
MEDICAL CONSENT POLICY
I hereby grant permission to the Department of Public Safety, or its authorized representatives, to furnish such medical care as my son/daughter, may require, including examinations, treatments, etc. This permission is conditioned upon the understanding that in the event of serious illness or the need for hospitalization and/or major surgery, RISD will use all reasonable efforts to contact me. Failure in such efforts, however, should not prevent RISD from providing such emergency treatment as may be necessary for the best interest in the life of my son/daughter. I also acknowledge that the Rhode Island School of Design must abide by both Rhode Island State Law and the individual policies of area hospitals with regard to consent for medical treatment for a minor. I understand that in the event of a medical emergency I may be contacted directly by hospital staff as necessary for the treatment of my son/daughter.
STUDENTS UNDER 13 PICK-UP + DROP OFF POLICY
In the interest of safety for all Young Artists, please follow the guidelines below.
Student Drop-off
RISD CE instructors may come to class early to prepare materials and set up the classroom. Please note that instructors are not able to supervise students prior to the start of class. Should you arrive early, please stay with your child until the class is scheduled to begin.
Authorized Adults
The only individuals authorized to pick up students under the age of 13 must be registered as an Emergency Contact online through the RISD CE registration site. Authorized Adults can be customized online through the primary's profile page or at the time of registration. We cannot release children to unauthorized adults. All adults must be prepared to show a picture ID to support authorization for pick-up.
Late Pick-up
We request the authorized adult arrive at the classroom promptly for pick-up. There will be a $20 fee charged for students that are picked up more than 15 minutes late. Student's whose parents are more than 15 minutes late will be taken to the lobby of 20 Washington Place, 1st floor, to wait with a CE staff member for an authorized adult. Consistent late pick-ups may result in dismissal from the program.
LEGAL GUARDIAN POLICY
All students under the age of 18 will need a legal guardian's consent to participate in a RISD Continuing Education class. If the person registering a minor designates that they are not the legal guardian, Continuing Education will email a Parent Guardian Consent Form to the adult registering the minor to be completed by the legal guardian. It is the sole responsibility of the party registering the minor(s) to have the legal guardian complete the Parent Guardian Consent Form. In order for the student to participate in the course(s), a completed Parent Guardian Consent Form must be returned to Continuing Education no later than one week prior to the first day of class. A student registered by someone who is not a legal parent or guardian without a completed Parent Guardian Consent Form on file before the start of the course(s) may be withdrawn.
